P0128 — Coolant Thermostat Below Regulating Temperature (5.9 Cummins 24-Valve (VP44))

Engine coolant does not reach the expected operating temperature in the expected time.

Updated September 2, 2026

What it means

The ECM monitors engine coolant temperature via the coolant temperature sensor and expects the engine to reach regulated temperature after a cold start under normal load. If the temperature stays below the calibrated threshold for too long, P0128 is set. On the 5.9L Cummins 24-Valve (VP44) this is a cooling-system/thermostat performance monitor, not a fuel or emissions fault.1

Most likely causes

  1. Thermostat stuck open or opening prematurely — A conventional wax-element mechanical thermostat that fails open prevents the engine from reaching normal operating temperature, causing prolonged warm-up and P0128. The platform uses a conventional wax-element mechanical thermostat.1
  2. Coolant temperature sensor reading low / biased cold — A sensor that reads colder than actual coolant temperature can keep the ECM from seeing regulated temperature. P0117 describes a low-input condition on the coolant temperature circuit on this platform.2
  3. Low coolant level or air pockets after service — Insufficient coolant volume or trapped air reduces heat transfer and can delay warm-up. Chronic coolant loss has been reported on this platform, e.g., block cracks leading to coolant seepage.34

How to diagnose

  1. Verify freeze-frame conditions: ambient temperature, engine run time, vehicle speed, and coolant temperature at set time.
  2. Monitor live coolant temperature from a cold start to operating temperature with a scan tool; compare to an independent temperature measurement at the thermostat housing.
  3. Inspect thermostat operation: with engine cold, remove radiator cap and observe coolant flow at idle as engine warms; flow should remain minimal until near regulating temperature, then increase.
  4. Check coolant level in radiator and overflow, inspect for external leaks, and pressure-test the cooling system to reveal seepage or internal loss. Coolant loss and block cracks have been documented on this platform.
  5. Test engine coolant temperature sensor voltage/resistance vs. temperature chart; inspect connector and wiring for corrosion or poor contact. Low-input faults are described for this platform.
  6. Inspect grille, air dam, and winter front for missing pieces that increase airflow; confirm fan clutch engagement is not excessive at low temperature.
  7. Verify coolant type and concentration with a refractometer; correct if diluted or contaminated.
  8. After repairs, perform a cold-start drive cycle and confirm coolant reaches and maintains regulated temperature without setting P0128.

Rough cost and difficulty

Parts cost varies by brand and region; labor depends on shop rates and access. Thermostat replacement is typically modest; diagnosis time dominates if intermittent. No emissions-related components apply to this platform.

When to see a pro

If coolant loss is present with no external leak, or if a cracked block is suspected, have the cooling system professionally pressure tested and inspected. NHTSA reports describe coolant loss from cracked blocks on this engine family. Seek a diesel technician experienced with 5.9L Cummins 24-Valve VP44 trucks for accurate diagnosis and to avoid unnecessary parts replacement.
