P0128 — Coolant Thermostat Below Regulating Temperature (6.4 Power Stroke)

What it means

The PCM monitors engine coolant temperature rise after a cold start and sets P0128 when the temperature stays below the calibrated threshold for too long. On the 6.4L Power Stroke this is commonly a thermostat that is stuck open or a cooling system that is over-cooling the engine. The platform uses dual thermostats and a radiator that is a known failure point.1

Most likely causes

  1. Thermostat stuck open (dual thermostats on 6.4L) — One or both thermostats fail open, coolant circulates continuously, engine never reaches regulating temperature. The 6.4L uses dual thermostats; a single stuck-open unit can still set P0128.1
  2. Cooling system over-cooling / excessive heat rejection — A leaking radiator or other cooling-system fault can keep coolant temperature low. NHTSA complaints document radiator leaks on this platform.234

  2. Compare ECT sensor reading to a known-good thermometer or infrared gun at the thermostat housing; look for implausible low reading.

  4. Command or observe warm-up profile with scan tool; on 6.4L with dual thermostats, check both banks reach regulating temperature.
  5. Perform a controlled warm-up test and watch ECT rise rate; a stuck-open thermostat shows slow rise and low stabilized temperature.
  6. If ECT is plausible low, remove and test thermostats for opening temperature and full closure when cold.
