What it means
The PCM is commanding a rail pressure, but the measured pressure is above that command. On this platform that points to pressure-control faults rather than a simple low-supply issue. The rail has a mechanical pressure-limiting/relief valve that can stick and cause high-pressure codes.12
Most likely causes
- Fuel control actuator (FCA/MPROP) stuck or not regulating pump delivery — The FCA meters high-pressure pump output. If it sticks closed or the control circuit is faulty, rail pressure can overshoot the command. Diagnose by checking FCA command vs. actual rail pressure with a scan tool and inspecting FCA electrical connector and wiring for damage/corrosion. Compare commanded vs. measured pressure at idle and under load.12
- Rail pressure sensor skewed high or wiring fault — A sensor reading high will make the PCM think pressure is above command. Verify sensor voltage/pressure correlation with a scan tool and compare to a known-good reference or substitute sensor reading. Inspect harness for chafing, moisture, and connector integrity.12
- Mechanical rail pressure relief/pressure-limiting valve stuck closed — The rail-mounted mechanical relief valve is a common real cause of P0087/P0088 on this platform. A stuck-closed relief prevents pressure bleed-off, leading to over-pressure. Diagnosis is by pressure behavior: pressure remains high even with FCA commanded low, and no change with FCA unplugged (per service info). Inspect valve for debris/contamination and rail cleanliness.12
- High-pressure pump over-delivery / internal fault — CP3 (2013–2018, 2021+) or CP4.2 (2019–2020) can over-pressurize if internal control is compromised. NHTSA complaints note high-pressure pump failures on this family. Check pump control and compare rail pressure vs. command across RPM/load; look for metal debris in fuel if pump has failed.123
- PCM pressure-control strategy fault — If sensor inputs and actuators test good but pressure remains high, the PCM’s pressure control logic may be faulty. Confirm with data logs of commanded vs. actual pressure, FCA duty cycle, and rail sensor values before condemning the PCM.12
How to diagnose
- Verify freeze-frame and repeat the fault under controlled conditions; record commanded vs. actual rail pressure, FCA duty cycle, engine speed, and load.
- Inspect in-tank supply pump electrical connections and confirm low-side supply pressure is within spec; low supply can cause erratic high-pressure control.
- Check rail pressure sensor voltage and signal integrity; compare scan-tool pressure to a calibrated test gauge if available.
- Command FCA duty cycle with scan tool (where supported) and observe rail pressure response; a non-responsive or inverted response suggests FCA or wiring fault.
- Evaluate mechanical rail pressure relief/pressure-limiting valve behavior: pressure should bleed when commanded low; persistent high pressure with FCA at minimum points to a stuck relief valve.
- Inspect high-pressure pump for signs of failure/debris; on 2019–2020 trucks note CP4.2 history and recall/replacement campaigns.
- Review related codes (e.g., P0087, P008A) and fuel-system adaptations; correct low-side supply issues before condemning high-pressure components.
