What it means
On the 7.3L Power Stroke the generic description ‘fuel injection pump fuel pressure regulator control circuit’ does not map to a fuel-pressure regulator because the engine uses HEUI injection with no common rail, no CP3/CP4, no high-pressure fuel pump and no rail-pressure sensor. Injection pressure is produced by engine oil via the high-pressure oil pump, Injection Pressure Regulator (IPR) valve and Injection Control Pressure (ICP) sensor. P1211 therefore reflects the PCM detecting the injection-pressure control circuit operating outside expected range/performance, i.e., an ICP/IPR system performance issue rather than a fuel-rail issue.1
Most likely causes

Faulty ICP signal causes the PCM to judge IPR control performance out of range. ICP/IPR faults are the platform’s injection-pressure fault family.1
- High-pressure oil system leak or aeration – internal/external oil leaks, bad O-rings, cracked standpipe/dummy plug, low oil level, or air ingress reducing actual ICP versus commanded — Low oil level or aerated oil also causes ICP faults on this HEUI system. Reduced actual pressure makes IPR control appear out of performance.1
