P0401 — Exhaust Gas Recirculation Flow Insufficient (L5P Duramax)

Diagnostic reference for L5P Duramax (2017–present) P0401.

Updated September 2, 2026

What it means

Measured EGR flow is lower than commanded on the L5P Duramax. The platform has an EGR valve + cooler; P0401 = EGR flow insufficient / low flow. Typical presentation is EGR valve stuck closed / carbon fouling, clogged EGR cooler / passages, or faulty EGR position / flow sensor feedback, often with accompanying EGR-related or intake soot restriction symptoms.1

Most likely causes

  1. EGR valve stuck closed or carbon-fouled — Valve pintle stuck closed or heavily carboned so commanded lift does not produce flow. On L5P the EGR system is present: EGR valve + cooler. Code P0401 = EGR flow insufficient / low flow. Typical on this platform: EGR valve stuck closed / carbon fouling.1
  2. Clogged EGR cooler / intake passages from soot restriction — Internal passages or cooler core restricted by soot, reducing actual flow despite valve command. Typical on this platform: clogged EGR cooler / passages, often with accompanying EGR-related or intake soot restriction symptoms.1
  3. Faulty EGR position / flow sensor feedback — Position or flow feedback reports less flow than commanded, setting low-flow detection. Typical on this platform: faulty EGR position / flow sensor feedback.1

How to diagnose

  1. Verify freeze frame and repeat the low-flow condition under steady cruise where EGR is commanded.
  2. Inspect EGR valve pintle and seat for carbon binding; check valve moves freely with command and returns to closed.
  3. Inspect EGR cooler and intake manifold passages for soot plugging; look for intake soot restriction symptoms noted as typical with P0401 on this platform.
  4. Check EGR position / flow sensor signal plausibility vs. commanded position and compare to MAP/MAF airflow model; MAF (intake duct) + MAP are used for EGR/airflow modeling per platform facts.
  5. Review related EGR codes and data; P042E indicates EGR control stuck open on this platform, which helps differentiate stuck-closed vs. feedback issues.

Parts and tools you may need

Rough cost and difficulty

Parts and labor vary by extent of carbon buildup and whether EGR valve or cooler replacement is required; diagnostic time is incurred before parts replacement.

When to see a pro

If cleaning does not restore flow, or if internal EGR cooler failure is suspected, have a qualified diesel technician evaluate EGR valve operation, cooler integrity, and intake soot loading. Do not attempt high-pressure fuel or safety-critical repairs without proper training.
