Most likely causes
- Thermostat stuck open or weak spring — A conventional mechanical thermostat that remains open allows continuous coolant circulation, preventing the engine from reaching regulating temperature. This is the most common cause of P0128 on these engines.1
- Coolant temperature sensor (ECT) reading low / sensor drift — If the ECT reports a temperature lower than actual, the PCM will think the engine is not warming up. Verify sensor accuracy against a scan tool and infrared measurement.1

- Inspect thermostat operation by monitoring upper radiator hose temperature; hose should stay cool until regulating temperature is reached, then warm rapidly. Do not remove high-pressure components.
- Check for continuous cooling fan operation or fan clutch engagement during warm-up; verify fan control commands with scan tool.
- Inspect wiring and connector at ECT for corrosion or damage; check reference and signal voltages per service information.
- If ECT reading is plausible but warm-up is slow, evaluate driving pattern and ambient conditions; repeat test on a longer drive cycle.
- Clear codes and perform a controlled warm-up drive to confirm monitor runs and passes.
- Document findings before replacing thermostat or ECT.
