P0191 — Fuel Rail Pressure Sensor A Circuit Range/Performance — LLY / LBZ / LMM Duramax

Diagnostic reference for LLY/LBZ/LMM Duramax (2004.5–2010) with code P0191.

Updated October 15, 2026

What it means

Generic definition: The fuel rail pressure sensor reading is implausible. On LLY/LBZ/LMM, compare rail pressure key-on engine off (should be near zero) and check the sensor and connector.1

Most likely causes

  1. Rail pressure sensor signal drift or internal failure — Sensor output is out of expected range for actual rail conditions; connector corrosion or oil intrusion at the sensor can bias the signal.1
  2. Wiring/connector fault to the rail pressure sensor — Chafed, oil-soaked, or loose harness at the rail sensor causes intermittent or biased voltage; inspect the sensor connector and wiring for damage, corrosion, or oil contamination.12
  3. Reference/ground supply issue to the sensor — Poor 5 V reference or sensor ground raises/lowers the signal and triggers a range/performance fault.12
  4. Actual rail pressure excursions due to fuel delivery problems — CP3 pump wear, pump-mounted regulator sticking, or fuel restriction can produce pressure behavior that the ECM judges implausible versus commanded pressure.1
  5. Contaminated or aerated fuel affecting pressure stability — Water, debris, or air in the low-pressure supply can cause erratic rail pressure readings that appear as sensor range/performance.1

How to diagnose

  1. Verify code and freeze frame; confirm P0191 is current and note engine speed, load, and rail pressure desired vs actual at setpoint.
  2. Key-on engine-off rail pressure check: with KOEO the rail should read near zero; a non-zero reading points to sensor/wiring bias.
  3. Inspect rail pressure sensor and connector: look for oil intrusion, corrosion, bent pins, and harness chafing along the valve cover/rail area; compare to source guidance to check sensor and connector.
  4. Check sensor supply and ground circuits: measure reference voltage and ground integrity at the sensor connector per service specs; repair wiring faults before condemning the sensor.
  5. Monitor live rail pressure vs desired pressure with a scan tool during idle and light load; look for implausible jumps, offsets, or dropouts that match the range/performance condition.
  6. If electrical tests pass, evaluate fuel delivery health: review fuel filter condition, low-pressure supply volume, and CP3 pump/regulator behavior using commanded vs actual rail pressure data (diagnostic observation only).
  7. Clear codes after repairs and perform a road test with data logging to confirm rail pressure tracks commanded values and P0191 does not return.
  8. Note platform specifics: LLY/LBZ/LMM use Bosch CP3 common rail with pump-mounted regulator and rail pressure sensor; injectors are external.

Typical US independent-shop figures; labor runs about $120–$180 per hour (dealers charge more). Confirm the diagnosis before buying parts.

When to see a pro

If rail pressure control is erratic under load, or if high-pressure fuel system service is required, have a qualified diesel technician perform the work. Do not service high-pressure fuel components without proper training and safety equipment.

Check for recalls

Recalls are tied to your truck’s VIN, so check yours directly: enter the VIN at NHTSA’s recall lookup. Open safety recalls are repaired free at a dealer.
